    目的 探究徐州市泉山区地区肺恶性肿瘤流行病学特征并分析其影响因素,以期为该地肺恶性肿瘤防控提供理论指导。方法 选取2019年1月至2023年12月在徐州医科大学附属医院和中国人民解放军陆军第七十一集团军医院确诊为肺恶性肿瘤302例作为病例组,选择同期体检健康者300例作为对照组,自设调查问卷了解患者的一般及临床信息,logistic回归进行危险因素分析。结果 两组吸烟指数(c2=40.058,P<0.001)、化工职业环境(c2=28.153,P<0.001)、每周运动模式(c2=11.021,P=0.004)、肺气肿评分(c2=142.812,P<0.001)比较存在统计学差异。此外,病例组CA125为(87.11±13.09)U/mL,对照组为(16.32±5.61)U/mL,差异具有统计学意义(t=34.581,P<0.001)。logistic回归结果显示,吸烟指数≥20支/天(OR=3.448,P=0.021)、化工职业环境(OR=4.091,P<0.001)、肺气肿评分中(OR=1.302,P<0.001),P<0.001)或重度(OR=1.461,P<0.001)、CA125检测值≥75U/mL(OR>1.6,P<0.001)为肺恶性肿瘤的独立风险因素,而每周中等程度运动(OR=0.821,P<0.001)为肺恶性肿瘤的保护因素。结论 吸烟、CA125指标过高、化工职业环境、长期肺气肿为徐州市泉山区肺恶性肿瘤发生的危险因素,应加强吸烟、肺气肿、化工职业环境就职群体的CA125定期筛检,并对适当运动进行健康宣教,以期预防和减少肺恶性肿瘤发病。

    Abstract:

    Objective To explore the epidemiological characteristics of lung cancer in Quanshan District of Xuzhou cityand analyze the influencing factors, so as to provide theoretical guidance for the prevention and control of lung cancer in Quanshan District of Xuzhou City. Methods A total of 302 patients with lung cancer diagnosed in the Affiliated Hospital of Xuzhou Medical University were selected as case group from January 2019 to December 2023, and 300 healthy persons were selected as control group, a self-designed questionnaire was used to investigate the general and clinical information of the patients, and Logistic regression was used to analyze the risk factors.Results There were a statistically difference in two groups for the smoking index (c2=40.058,P<0.001),Chemical occupational environment( c2=28.153 , P<0.001), weekly exercise mode(c2=11.021,P=0.004), emphysema score(c2=142.812,P<0.001). In addition, the CA125 in the case group was (87.11 ± 13.09) U/ml, while the control group was (16.32 ± 5.61) U/ml, with a statistically difference (t=34.581, P<0.001). The logistic regression results showed that smoking index≥20 cigarettes per day (OR=3.448, P=0.021), chemical occupational environment (OR=4.091, P<0.001), emphysema score (OR=1.302, P<0.001) or severe (OR=1.461, P<0.001), and CA125 detection value ≥ 75U/ml (OR>1.6, P<0.001) were independent risk factors for lung cancer, while moderate weekly exercise (OR=0.821, P<0.001) was a protective factor for lung cancer. Conclusion Smoking, high CA125 index, chemical occupational environment, and long-term emphysema are risk factors for the occurrence of lung malignant tumors in Quanshan District of Xuzhou City, screening regularly of CA125 in people who have smoking, emphysema, and being chemical occupational environment should be strengthened, and appropriate health education in proper exerciseshould be provided to prevent and reduce the incidence of lung malignant tumors.
